PROGRAMS

Some of the programs we offer are Administration, HIV testing, Early Intervention Program (EIP), Bridge, Transitional Case Management Program (TCMP), Education and Prevention (Outreach), Surveillance, Mental Health, and HIV Transmission Prevention Program (HTPP).

Early Intervention Program (EIP)

  • Nursing Intake
  • Psychosocial Assessment
  • Health Education
  • Nutritional Assessment
  • MD Intake
  • EIP staff provides a support network to address emotional and clinical needs

Transitional Case Management Program (TCMP)

  • Offer pre- and post-release assistance to parolees
  • Act as a liaison between prison staff, local service providers and parole agents
  • Contact inmates prior to parole dates to discuss a service plan and provide them with initial information and referrals
  • Continue to assist parolee during the first 60-90 days after parole
  • Link the parolee with community case management for long-term follow-up care

HIV Transmission Prevention Program (HTPP)

  • Provide harm reduction psychotherapy and case management to high risk HIV+ and HIV- individuals who are at risk related to drug/alcohol use and/or sexual behavior
  • Client centered counseling
  • Identify barriers
  • Goal –Prevent HIV transmission through individualized counseling sessions

Community Health Outreach Workers (CHOWs)

  • Reach communities at risk in their environment
  • Link between the community and resources
  • Provide free testing and counseling
  • Education and Prevention
  • NIGHT-Neighborhood Intervention Geared to High Risk Testing

Bridge Program

  • Re-activate old case files
  • Assist recently diagnosed HIV+ clients into care
  • Provide support for living a healthy and independent life
  • Provide referrals ex. transportation, housing, education, food programs and mental health

Mental Health

  • Provide education, prevention and testing for substance abusers
  • Provide education and prevention for youth at risk

Surveillance

  • Report to the State Office of AIDS
  • Gather HIV/AIDS statistics
  • Ensure all providers are kept current of HIV regulations and reporting to SOA
  • Provide partner notification
